Keys to Successful Survey Administration
Proper administration of the Healthy Youth Survey is essential to ensure the integrity of the data. Improper administration may compromise the data, and consequently you may not receive a local report for your school or district.

Please remember the following keys to proper administration:

  1. Carefully read and follow all instructions.

  2. Attend all required trainings, even if you have administered the survey before. (Procedures change from one administration to the next.)

  3. Keep all survey materials you receive handy. You may need to refer to them later.

  4. Check the HYS04 web site for updates, reminders, tips, and answers to frequently asked questions.
